Air Ambulance Operations in South Asia: Critical Care & Fast Track Permits

In medical aviation, the “Golden Hour” isn’t just a concept—it is the difference between life and death. When a patient needs to be airlifted from Kathmandu to a super-specialty hospital in Delhi, or repatriated from Mumbai to London, there is no time for bureaucratic delays.

At Aileron Air, we treat Medical Evacuation (Medevac) flights with Humanitarian Priority. We understand that a Medevac is not a standard charter; it is a critical care operation that requires a completely different set of protocols, speed, and precision.

This guide outlines exactly how we expedite permits, secure tarmac access, and manage the complex logistics of air ambulance operations in India and South Asia.

How Fast Can You Get a Medevac Permit in India?

How Fast Can You Get a Medevac Permit in India?

While standard landing permits take 3 working days, Air Ambulance permits are exempted from standard wait times.

The Reality: With the correct documentation and immediate manual follow-up with the DGCA (Directorate General of Civil Aviation), Aileron Air can secure landing permits in 2 to 4 hours, 24/7/365.

Important: To achieve this speed, the flight plan (Field 18) must explicitly state “STS/MEDEVAC” or “STS/HOSP”. If this status is not correctly filed, the automated system will treat it as a general aviation flight, causing potentially fatal delays.

Tarmac Access: Getting the Ambulance to the Aircraft Steps

Tarmac Access: Getting the Ambulance to the Aircraft Steps

The most critical transfer point is moving the patient from the ground ambulance to the aircraft. Standard security rules prohibit outside vehicles from entering the tarmac (airside).

We Bypass the Standard Rule.

For confirmed Medevac flights, Aileron Air arranges BCAS (Bureau of Civil Aviation Security) Special Permissions to allow the ICU Ground Ambulance to drive directly up to the aircraft stairs.

  • Why this matters: It eliminates the need to transfer a critical patient onto a wheelchair or stretcher through the terminal building.
  • Seamless Handover: The medical team on the jet can interface directly with the ground doctor without breaking the “sterile” medical environment.

The “Green Corridor” Advantage in Delhi & Mumbai

Landing the plane is only half the mission. The patient often needs to reach a hospital like Medanta – The Medicity (Gurugram) or Apollo Hospital (Delhi) immediately.

Traffic in Indian metros is notorious. For critical cases (e.g., organ transplant or cardiac emergency), Aileron Air coordinates with the Traffic Police to establish a “Green Corridor.” This involves manually controlling traffic signals to provide a non-stop, cleared route for the ambulance from the airport exit to the hospital door.

Checklist: Mandatory Documents for Urgent Medical Permits

Mandatory Documents for Urgent Medical Permits

To trigger the “Urgent” protocol with the DGCA, we need the following immediately. Do not wait to gather these; send them as they become available.

  1. Fit-to-Fly Certificate (FTF): Issued by the treating doctor at the origin hospital.
  2. Hospital Admission Letter: Confirmation from the receiving hospital in India (or destination) that a bed is reserved.
  3. Medical Summary: A brief report on the patient’s condition (required to justify the urgency to the DGCA officer).
  4. Passport Copy of Patient: Clear scan.
  5. Attendant/Doctor Details: Passport copies of any family members or medical crew flying on board.

Regional Hubs: Where Should You Land?

Regional Hubs: Where Should You Land?

Not all airports are equipped for critical transfers. We recommend the following hubs based on medical infrastructure:

  • North India: Delhi (VIDP). Best for proximity to world-class hospitals in Gurgaon.
    • Alternative: Jaipur (VIJP) if Delhi has severe fog/smog diversions.
  • West India: Mumbai (VABB). Excellent cardiac and cancer care facilities.
    • Note: Mumbai has severe slot constraints. We often negotiate “Medical Emergency Slots” which override commercial traffic.
  • South India: Chennai (VOMM) or Hyderabad (VOHS). Both are major hubs for medical tourism and transplant surgery.
  • From Nepal (Kathmandu): We handle daily evacuations from VNKT to India. We know the specific “drop-and-go” procedures required due to Kathmandu’s lack of parking.

  • 24/7 Human Dispatch: You never get a voicemail. You speak to a dispatch officer who understands medical terminology.
  • Oxygen & Equipment: If your aircraft needs a refill of medical-grade oxygen (O2) or ground power (GPU) to keep onboard incubators running, we have it ready upon arrival.
  • Compassionate Handling: We assist distraught family members through Customs and Immigration with priority, ensuring they are not separated from the patient.
